Would you say Elthina is to blame for Kirkwall's problems in Act 3 eg failing to be a proper source of leadership and authority, letting Meredith act unchecked and failing to deal with the issues between her and the Mages ?

I think Kirkwall's problems were based in long-standing structural weaknesses combined with the unconciliatory mood of everyone involved. She's not to blame, but she certainly didn't do anything to help the situation.

I agree, thats actually more what I meant, not that she caused everything but didn't help by been neutral and allowing Meredith to create a Templar police state and abuse the Mages.

True that. To be fair to her, we don't know how much power local Chantry clergy have over the Templars -- it might be very well possible that the knight-commanders answer only to the knight-vigilant, who in turn answers only to the Divine. The Divine, of course, was informed of what was happening in Kirkwall and sent the Seekers to investigate. But I daresay Elthina could have spoken up during the inquiry and led to a more mage-friendly conclusion. Add to that the personal influence and authority she seems to have over Meredith and Orsino. And Kirkwall as a whole; if I recall correctly, she may have been behind the assassination of Viscount Perrin Threnhold.

At the end of the day the conflict sparking was Ander's fault, he had a really good point and might even have been right about talk not achieving anyway but thanks to his own anger and outrage and Justice's impatience (spirits after all find time hard to deal with as a concept given how different things are in the Fade) making him a reckless and irresponsible terrorist. Though to be fair I doubt even he though Meredith would suddenly declare the Right of Annulment given how the Circle wasn't involved at all.
She's a good example of what happens when a hateful fanatic gains power given she both hates mages/despises them and is very devout. Although her use of the Tainted Lyrium Sword marks her as a hypocrite since it gave her magical powers of her own to a degree.

I think that was exactly what Anders was going for. He knew Meredith would invoke the Right of Annulment, and was counting on the uproar that would cause in the other Circles. That way, he'd sacrifice Kirkwall and himself while still giving the other Circles a good reason to revolt. Of course he wasn't hoping Kirkwall's Circle would fall, but he got himself into a situation where he'd win one way or the other.
That's the old contentious point of whether Templar abilities constitute magic or not, in a sense.
