That's a studio setup, some details based on EXIF-data:

Model - Canon EOS 5D Mark II
ExposureTime - 1/125 seconds
FNumber - 3.50
ExposureProgram - Manual control
ISOSpeedRatings - 100
FocalLength - 100 mm

I don'r remember the exact light setup, but based on the image and my usual setup it looks like the main light is on the left and one reflector in front of the model close to the lens. Lens was the EF 100mm f2.8 L IS USM Macro.
