I think I've figured out why this is so scary, at least in its execution. First, you have the Uncanny Valley factor, especially on the teeth; this has been described as "putting realistic textures on things without realistic proportions". Second, and what probably overlaps with the Uncanny Valley, is his extremely human teeth. Those dentals don't belong on anything that isn't human -- no human features in detail belong on anything that isn't human -- and that is why muppets with human eyes, that one movie poster from a while back about a guy in the body of a dog which showed the dog with human eyes, and monkeys creep me out. (Seriously, monkeys are SCARY when you look at them for a while.)
TL;DR his teeth are rendered in incredible detail, and they're also human teeth on something that isn't human.
I tested it by putting my finger on the screen, and it really isn't a nightmare-inducing picture any more with his teeth censored out. But I have to say, good job on getting that reaction out of me. I'll do my best to forget about it before I go to bed.

I just love drawing teeth and smiles, especially making plain human teeth look intimating instead of going the carnivorous route and making them all sharp. I went wild when sketching and shading the teeth and just pushed them to be a focal point instead of an extra detail. Also, I think using horses teeth would look a bit silly with a malicious grin, so I decided to forgo a bit of realism for the sake of reaction. Glad to know it worked, happy to know you like the picture, and I apologize for any nightmares. XDD
