I like the purity, simplicity, and vulnerability here. I feel the tree as veins, like the simple fragility and ephemerality of being human. And the hand is held like it is somebody begging for something, or giving up something unseen.
But I'm probably just reading too much into this.

I overlayed a photo of a tree onto my hand, and erased other distracting elements. That's really all there is to it, nothing special!

It's a double exposure. This guy has nice camera tutorials, but I haven't watched this one:

You can do it in programs other than photoshop, like GIMP and SAI if you get creative and know the program well, I believe.

This is a really cool shot! I love the overlay here as well as the
juxtaposition you have going on. I kinda feel like there should
be something in the hand though, perhaps a ruby red apple.
It would have tied the hand even further to the tree and given
a strong subject :3 Just a thought... I really like this though,
