Hum...I don't think that Ai's surname is "Ai", in fact the name "Enma" cames from the name of the lord of hell of the japanese mithology (it's also mentionen in other animes). They also call her "Ai" before she became the Hell girl, so we can say that the lord replaced her original surname with his name, keeping her first name untouched.
