Depends what program you're using. In MangaStudio (which I use) they're added automatically depending which speech bubble you use. In Photoshop you can add it by having the bubble in its own layer and then using 'stroke' from the layer effects. Not sure about other software though.

'tis good! The full version's quite pricey but there's a much cheaper one that doesn't have all the bells and whistles but still does the speech bubbles and useful stuff called MangaStudio debut.
