Personally, I think at some point MJ emotions overcame him and as a result he started to neglect his mental health. His sleeping disorders and pain where sure signs of some mental health problems and nobody saw the writing on the wall. The ones that did just ignored it (overlooked it), perscribed pain meds and made their money.
I'm sorry to see him go like that.
