Actually, I don't know if anyone's said this, but milkdrinker is probably referring to breastfeeding. That you're so weak you're still at your mother's breast drinking her milk. That's how I always took it at least.

A possible reason could be because the Nords needed something to insult the Imperials with, and "They're nuts, those Imperials" in best Obelix-style, apparently wasn't good enough. So, since the Nords drink so much more mead and ale, they just picked the least drunk liquid and used it for the insult. At least, that's my tale on it...
