Initially I thought the focus was too far on the palm, but then it all made sense... It's a diversion. As such things often are. To distract from the overwhelming pain making it livable, putting it in the background. I interpret the hand as a sort of play on words, a visualization of "the matter at hand" meaning the whole focus. Nothing else matters in that instant, not the wall, not the table, because in this moment everything is either unimportant or a reflection of the pain. That's why we focus on the distraction... to survive it all.

I think that had the focus been more on the fingers the image would have been better.
aside from that the reflective surface adds alot and in some ways balances out the background.

Agreed, especially from an artist who's works could be summarized as "fashion" shots by anyone choosing to overlook anything substantial about them.
Makes a person wonder though: what deeply personal reason brought such a reaction? Something in your image must have spoken to a place she didn't want to hear.
