I use OpenCanvas, and basically just layers. On one layer I put flat colors, which is the layer the spots are on. Another layer I blend shades of grey together with a watercolor brush to get that shading effect. I draw with a tablet, which senses pressure, allowing the line you draw to be bigger when you press hard and smaller when your soft. That's what I use to make the spots. I select a fairly small brush size, then I just tap like crazy, harder taps make bigger spots etc. And since its on a different layer than the shading, it doesn't interfere with the shading technique I use.
