I found that the use of bleach in this to be the most striking aspect of it. I'm guessing that it was used to make the splotches on the top of the bakcground. If not, those splotches I think is the element that adds contrast since the texture of her skin is the same as the texture of the background (the dotted shading/coloring). They really make her face, neck and hands stand out more.
