This skin is meant to depict Doomguy as a combat medic, fully armored and equipped for treating all sorts of battlefield injuries, from civilians to military personnel and even augmented supersoldiers.
His white medic armor is designed for easy recognition and high battlefield visibility. Instead of weapons, this version of Doomguy would carry powerful medical equipment that can be repurposed and reprogrammed for self-defense or offensive purposes (similar to how Dead Space's gameplay features industrial tools).
For example, the Doomblade takes the form of a plasma Mediblade designed to remove injured soldiers' armor, but which can just as effectively be used against enemy armor. The shoulder launcher is a multi-purpose Meditool combining a hyperspectral medical scanner, syringe gun, and healing grenades, but can just as easily be used to scan enemies' weaknesses, launch toxic darts, or deliver targeted biochemical ordnance.

This concept is actually really interesting, as a combat medic that can synthesize battlefield antitoxins and healing agents is just as capable of producing biochemical weapons. Stasis tools used to stabilize injured patients can just as well immobilize opponents. Rescue tools that can cut or even burn through rubble or wrecked vehicles, like saws/drills and even thermite, would be even more effective against combatants. And if they can diagnose and repair patient augments and software, they just as easily might disassemble and cripple enemy cybernetics.

This skin was greatly inspired by the Space Marine Apothecaries from Warhammer 40k.
