The proportions are mostly okay, and I like how you didn't just use dodge/burn for the highlights and shading in some places.
I have noticed some pillowy-shading in your newer works as well. I hope I'm not sounding rude... really, this is an awesome picture, and your other works are cool too. I just recommend drawing an arrow and then using it for the light source and direction. Your hands seem to be mostly ok as well. Keep practicing!
