Do you base these on actual people or is it from scratch?

From scratch with referencing certain things I might be having a hard time with, like how different lights hit the skin or understanding a nose at a certain angle. I will usually use myself or friends because it helps the best.
Although sometimes in the end, people have remarked that they look like other people, then I can't un-see it! :b
