In RomaHeta, Italy as well as some other characters get sucked into an online game and have to play their way out. During the game some players, Italy among them, get attack by a virus, causing them to turn on the others. Holy Roman Empire's ghost saves Italy, putting him back on the path of good. My guess is that was what was happening here.

romaheta is an RPG. u can watch it english subbed on youtube or read the fanfiction. (i prefer the fanfiction) idk if u can play it though. i don't think it is in english.

uuuuhm... i think hre would italy no matter what xD even if he found out italy-chan was a guy... xD i mean he is so obsessed with him, there is no way he could abandom ita-chan ;_;
(and i think in romaheta hre already knows italy to be a guy xD since this is italy and not chibitalia? hre is something like a guardian angel to italy in romaheta, and in the scene i drew he saves italy from being deleted ... i didnt really understand the story of romaheta yet since my japanese is poor orz)
