Cleansing the background in Photoshop can be a fairly easy or difficult process, depending on how smudged the paper is when you scan it in. Generally, though, it's simple enough to do. Here are two ways I clean (and darken) my images*:
1. Open up Photoshop and then open up the image you wish to cleanse.
2. Go to "Image" > "Adjustments" > "Levels".
3. Underneath the "Input Levels" box, click and drag the little arrows to change the brightness and contrast of both the image and the background.
4. Click "OK" when finished.
Optional:
1. Go to "Image" > "Adjustments" > "Brightness/Contrast...".
2. Use the "Brightness/Contrast" scroll bars to change how dark or light you want the image to be.
3. Click "OK" when you're finished.

*Take into account that I use Photoshop CS2 when touching up my images. The location of certain tools may differ from one version of Photoshop to another, so keep in mind which program you're using.
