A real film scanner will get you a sharper image; however I've made 16x20s that were very good looking with my nose to the print so I haven't justified the extra cost.
For 35mm I have a real Minolta film scanner and I can really see a difference with that when using slides. With most B&W I prefer the Epson because the Minolta will show every bit of grain on the negative where the epson doesn't.

I just scan the film. I have been using an Epson V700 and it has served me quite well.

Ditto. The only thing for me is that I am not quite sure sometimes whether my scanner is out of focus, or it is my lens.

The height might need to be adjusted and the negatives not setting flat may be a bit of a problem as well.
