from a symbolism point of view, the large figure represents harry the smaller one represents Heather/Cheryl. Its sort of father and daughter tbh. It can also represent Vatiel and the "god" being that Cheryl was supposed to have.

The larger figure appear to have slight boobage... It could be Harry Mason and Heather, or it could be reffering to the fact that Heather had grown up without a mother and as such, has an instinctual yearning for one. The fact tha tbeing is clinging to the child might represent the deep caring and attachment that is said to be beyond death itself.

The fetus within is creating the 'otherworld' that manifests around Heather, which are the memories of Alessa, who was meant to give birth to The God in Silent Hill 1. Maybe this image is a manifestation of Alessas desire to 'save' herself from dying, which she does at the end of SH 1 by giving Harry the newborn baby, Heather.
