In japan, Poison is a male to female transsexual--a transwoman. In America, she's a transvestite. The reason behind these choices was to avert any attention from feminists who would cry violence against women. Which is rather silly in this day and age since there's many games with female combatants. Technically neither case would constitute her being a man, as both instances she is said to identify as a woman.
