It happened to me twice, and I've seen many people do it without noticing.
It seems that it happens only on the mobile version of the website.

Well, it.s not the user's fault, the mobile version of the website does that sometimes and replies to someone instead of commenting. It happened twice (at least) [Edit: it just happened right now in this deviation XD ] for me and I've seen many people having the same issue.
With that said, you might want to check your works from time to time to not accidentally ignore a comment sent t the wrong person and not appearing in your inbox.
