How do I open it after I download/extract it because it comes up as an .rar file

You just have to download the rar:file and convert it with "winRAR" or "7zip" (for example).
To load a pattern set, go to the Patterns palette (in the fill tool, Pattern overlay style, etc.), then click the small arrow in the top right corner and choose "Load Patterns". You can also load patterns via the Preset Manager in Photoshop 6 and up.
