'k, the way I see this image is - girl in the bottom is the actual protagonist, physically; she is either very badly hurt and unconscious or even dying. The identical girl in top is representation of her spirit/conscience, who is shielding her from trauma and/or helping her trough pain/death. I just hope there's a medic nearby.
