It's because of a combination of cancer and his healing factor. Because of Weapon X experimentation he ended up with the latter, and he's been disfigured because he heals faster than his cancer can kill him. One theory for why he's so insane and breaks the fourth wall is because his condition drove him there.
In very recent comics he's been cured, but also doesn't have his healing factor.
