I can never figure out where little highlights like on the foot and in the middle of the arm should be. How do you decide where little fragments like that should go?

I put my imaginary light source and follow it in my artwork. Or, I just do it in the most plausible way, because it looks good; that arm shadow oppening is not accurate but I liked its initial shape so I left it untouched.
For more complex stuff you can use models and maquettes.
