It's an icon of the charity organization the International Order of Odd Fellows Lodge #12. The chain stands for the Three Virtues (Friendship, Love, Truth). Anything else, google or use an encyclopedia.

The chain of three links is the most common symbol of the OOF and it is the most popular organization outside of Masonry in most cemeteries.

"IOOF" is the International Order of Odd Fellows, a fraternal organization. Not as well known as the Masons. This is only the second one I've ever seen. I don't know what the"FLT" (or is that an I?) stands for?

It could be, the one I saw was not that elaborate. Unless the darn thing had sunk so far into the ground.
