I thought it was appropriate that Jim, as corny as it seems, was McCoy's personal star.
In the fiction, I felt that not only did Jim fufill his promise of literal stars to McCoy, he also became McCoy's own special something. Even though the other stars in his life may slip away from his grasp (however still shaping who he is today), Kirk will remain his constant and his own little star to keep. And vice versa.
The line that this picture itself is strongly based off of is, "All McCoy could see, all that could fit inside his eyes and his mind and his heart, were the stars."
The stars being Joanna, Spock (+crew), his childhood, the Enterprise, and hopefully, Jim. I wanted them to be placed intimately not only for the sake of slash, but also because the stars are so sacred to just the two of them as they may be the only two who will "have the stars as no one else has had." And of course each other as no one else has had. (Guh. Sorry, I'm so terribly corny.)
My interpretation may be totally off (and my explanation on crack) and my drawing totally irrelevant, and if so, I deeply apologize. I haven't had a lot of fiction reading time lately and I've tried to dedicate some time to analyze the writing to make an appropriate drawing. I know the picture is unjust for a great piece of fanfiction and/or an amazing artist (lastshadow).
