Hm...in that case, he/she needs to get the act together and say soemething! But if you want more, subtle, maybe the savior kinda meantions it? They could fight or something next time there's danger.
Plus, the Save-ee will probably be a bit scarred from the fact his/her life was also taken. You could put them thinking about it over and over, and finally thinking. "why DID they save me...?"
usually people get the hint after a while, though. I think evenutally the person who was the savior would just get frustrated and blurt it out even, without meaning to.

Well, I think the pain reason that he dislikes her is because she kinda tried to push herself into his life, and he doesn't act natural. And he finally confronts her on this when she saves his life AGAIN, along with his siblings. I didn't want to guilt him into being her friend, or make him feel bad. I wanted him to respect her.

    If you want him 2 respect her, make her do something for him that no one else will. Proving her strength is good too, but…maybe she finds out something about him? Or helps him in some way, when he wanted help but was afraid to ask for it. Or even him catching her doing things her way, not taking anyone’s rules. Things like that will make him slowly admire her, see her strength for what it truly is.

Eh,

When your trying to act strong, you can hide what you feel from some people. But people who really know here – especially someone who loves her, or someone she loves – would be able to see through that without much work. When you know someone, inside and out, no mask in the world can hide what you truly feel.

Hm, that's a good idea... can you tell me HOW they saved his/her life?

But here are some rough ideas;
They could get into a argument or some kind, and the one could bring it up; save when the save-e was about to do something reckless. “You can’t do this!”

    “I can do what I want! I don’t need someone like you to watch my back, I can handle myself!”

    “I almost watched you die! In front of me!” and then they say how much they care in kinda a fit of emotion.

    Other ideas could be the one who was saved having nightmares about that danger, thinking of it over and over until she/he finally got a clue.

    You could keep writing, and see where it goes. Time goes on, things change. Even if they don’t realize that she/he is in love with them, they WILL grow closer to the person. It’s not possible to be saved by someone, on that level, and not grow closer. Grow to care about them a bit, and finally, wonder WHY THEY DID IT? Why did they go to all that trouble, risk their own life, to save mine?

    Though it might not be as dramatic, slow realization can work. If you use this method, put a more dramatic ‘tipping point’ in, where he/she finally has the EURIKA! Moment, ya know?

    Or she/he could figure it out quickly, but try to hide it for a while. Sometimes when your in love, you just can’t believe it or face it yet. Ah, Denial, the classic symptom, no?

    The one who saved the person might also finally realize just how much this person means to him/her. Perhaps they had been on the edge of admitting to her/him he loved her, but scared? Something like this, the fear of losing that person for good while being able to do nothing but stand by and watch…that can shock him into just blurting it out, too.

Spear'd. XD right in the stomach. idk if I'm actually gonna put it in, I was just tossing random ideas, and it might just actually end up being a hologram... BUT there will be some kinda saved thing.

just based on his personality, I figured he doesn't even care after that. like, 'idiot, you actually think that would've hurt me? pathetic'
but I think that's only because of how badly she ended up pushing him away, accidentally.

but I doubt he actually 'likes' her likes her XD and I doubt that she would ever let herself reveal any emotions, especially with what happened in her past.

actually I think I figured it out! maybe... the second time she saved his life, but this time it's not just him, it's his whole family. and she gives him the opportunity to work together with her, so she doesn't feel like she's taking his place... maybe. but I'd still like to hear your opinions. ^^

Pfft, ok. Insane people usually have moments where they are perfectly sane, but can snap at the weirdest things. Make them change moods a lot; happy and calm one minute, and shouting and murderous the next. They go off the rocker. They'll smile at someone one minute, then shot a henchman for fun the next.

Smiling at weird points, too. Oh, and a really creepy thing to do? If your hero is being chased by the insane dude, or tortured or something, it's super freaky if they start humming a tune. Like, your hero's running up and down dark streets and hears your villain singing "*hero's name here* is faling down, falling down, falling down..." *evil laugh. Make his voice echo at perfect times, too. 

Google some old nursery rhymes for him to mumble, some of them are really disturbing if you listen. Like, Ring around the rosy is about small pox deaths.  

They also enjoy things that normal people would find disturbing. Give him/her a strange habit or hobby. You can Google some Physchogoy stuff too. For example, he could be a Lyncathrope; a disturbed man who takes on wolf-like characteristics.

Oh, and for the insane killer types, they love 2 take trophies. Your hero could find a room full of these 'prizes,' which could be anything that reminds the villain of his former victims.

A weird laugh is big, though. Snorting when he laughs, maybe. Talking to himself is good but take it farther. Make him have whole arguments with himself in DIFFRENT voices, apologizing to himself, giving himself shit, etc.

His character design is big, too. Messy, greasy hair, and greedy, wild eyes.

He's pretty hot-tempered and narrow minded. He hits first and asks questions later. He's not very smart, and he usually spends his time beating ppl up. Typical bully ^^ he'll beat up anyone who insults him and doesn't care for most other people

Hm... ok, I think I get it. Make him shout at lot, like his temper is what's controlling him. You could make a little creepy if you want; like he looks at the blood from a guy and smiles at it, or laughing as he punched someone.
If ur drawing him, do crazy eyes, for sure! Make the whites of the eyes big with small pupils, and make the eyes diff sizes as if he's almost got a twitch.
If he's not very smart, is he the henchmen type? You could writing him asking the boss to let him beat up someone the same way a kid would ask for Christmas presents. "It won't take long, just a little blooding up...it'll make him talk more, go on, i'll be fast!"

And for sure, put in a fit of rage or two. He gets so angry, screaming and smashing everything in sight. Maybe make him loose a fight to ur hero, and then go home and just beat the crap out of his own allies. If your hero is beating him, make sure to put his nger getting higher and higher as he loses more. He'd probay not have a lot of skill in fighting, just smashing through like Red Hulk. Laughing as he sat on someone's chest and beat their faces in, u know?

The one problem with that is that he's chained to a wall. He has restricted movement. But I do make him lash out at the others in an attempt to attack them.

I Dunne about that, then. Crazy people will sometimes get weird when captive. Like, they'll yank at their wrists until they bleed trying to get away. If he's alone all the time, he's probably talk 2 himself
