First, the color temperature is well balanced, the bright red pops, and the tan allows the picture to rest. All of the lines move toward the center of the image. Good gesture of the negative space. The tree adds a vivid element to the scene.
There is an element of surrealism, and psychological realism at the same time. You can picture yourself as that person, the question is "What brought you to this". The different shades of blue on the body are nicely applied to give a sense of light logic, and having several layers of shadow enhances the posture of the woman.
The concept has been done before, and you do it justice.

I wish I'd commissioned this - I'm writing a story and THIS would have been the illustration for it - but the story is nowhere NEAR as good as this picture. And it's clever for only using three colors. Wonderfully done!
