I was having a similar thought myself after seeing the movie, but I believe that the timeline of the movie starts, at least for the 'good' Doctor, roughly a week after Civil War, and ends about half-way through Ragnarok.
EDIT: just got the mental image of the weaponised shipping... Oh my...

Before Strange's accident, one of his potential patients was a man in the military with a spinal injury from an experimental suit, if that was meant to be Rhody in secret (because of the accords) then that'd put that scene at roughly a week after Civil War if not directly afterwards. Then we're shown a montage of Strange's various surgeries, his travel to Nepal, & his magic lessons which would've taken about 6 months, at the earliest, and then the scene with Thor outright had Loki mentioned when at the end of 'Dark World' Thor had thought Loki dead, meaning that the first part of Ragnarok, or at least a flashback, needs to show Thor discovering Loki's deception. Anyway that's my theory on the matter.

I think Loki was already half out of his mind with shock and pain and rage and self-loathing when he let himself fall from the Bifrost, and I have no doubt that his time in the Void broke him still further. Then he ended up with Thanos and the Other, who used physical torture, psychological torture, and literal mind-control to destroy and rebuild him, over and over again, until there was just a thread of the real Loki left, buried beneath the puppet they had created. So Loki had no true choice when it came to invading Earth, but there was enough of the real Loki to subtly influence his actions. Which is why Loki lost to the Avengers: he was TRYING to lose by using tactics which made no rational sense. He wasn't able to throw off Thanos' control enough to openly defy him and refuse to fight at all, but he could choose self-defeating strategies which would let the humans win. Of course, Odin is a blindly arrogant #@%^$ ! who never even bothered to ASK Loki what had happened to him after his fall, or why he'd invaded a realm he'd had no previous interest in, so Loki just focused inward during his imprisonment, fighting to truly recover his own mind and soul. Not that any of his supposedly loving family gave him much help with that.
