umm, mostly the way I do it trace the outside with the pen tool, then fill in the rest. It might be cheating, I dunno.
I could tutorial-ize it some point if you wish.

The sorta "cut and paste" border is a really neat effect. O_O I've seen it around and about but never really bothered to try it.
