I don't think I'v ever had a clear understanding of the meaning of this image. I may suggest that it is a trepanning to unblind the third eye, to reach enlightenment or something. The five nails around the hole in the forehead were drawn to get a star shape, but I don't know what they may be for in the context of the picture. Presumably, it is a part of some dark ritual. But all of these reasonings are shallow. There should be something else. Something deeper. Or not.

Perhaps it is just a catalyst ... that causes one to ponder.
One who can stimulate wonderment in others is endowed with a unique gift ... similar to the ability to make people laugh.
Do you spend much time in contemplation of things?
