In a comment Horikoshi made regarding some of the recent chapters, he mentioned that the reason Deku looked so scary in the pages because he was covered in blood and dirt (along with his tattered clothes and multiple quirks.)... You can understand why people thought he was a villain.
Can you imagine finding your friend in that state? I can't.
