Gives a whooole new meaning to having an eye in the back of you head ^^ Okay, that was lame, I know, but I can't find anything to put into this comment that won't be like the others.. I'll try though..
The shading is freaking amazing, and i love the vieny parts flowing free around the eyes. I love how it looks like some sicko scooped out this guys eyes and placed one in the guys mouth, and carved a new eye socket for him in the back of his head, and that once that happened, the skin around it formed into proper eyelids and everything adjusted to it. I love the shines on the forehead and the nose, as well as on the eyes, and that the picture has no definate black, blocky outlines to it, making it look more natural and amazing. I love the blood-shot eye in the back of his head, and the glazed expression on both of the eyes.
A truely interesting and fascinating picture.

Those stray hair type things remind me of the illustrations in Scary Stories to Tell in the Dark. Did you ever read those books? They were possibly the most traumatizing experience in my childhood, but I kept going back for more.

I've read those, they were awesome.
This one remids me a bit more of the bumps the Sci Fi channel does durring the commercials, though. Have you seen those? They go; shows something semi-normal, then freaky, then makes it normal, followed by "Sci Fi" text on screen, some letters fade, then it says "i F". And they're pretty cool, too.
